There is hope. I believe no matter who you are or what you have gone through, there is a light at the end of the tunnel, even if you cannot see it. With that said, I understand there are challenges we all face. And sometimes the challenge is asking for help, while other times it is in the process of healing from painful experiences. Still I’m confident, no difficulty is too big or too much and no matter who you are, you are worthy of love and compassion.

Case management is meant to empower you as a whole and enable you forward- by building skills, connecting you with support, and helping you find your voice. As your case partner, l hold hope even when you may not see it and advocate for you when you may not feel as if you have a voice. While I cannot change the past or what you have gone through, I can help you walk through this season of your life with practical assistance towards living the healthy life you want.

Our first meeting together will focus on gathering information. Together, we will review processes establishing your care and discuss your rights and our responsibilities so you understand everything about your services. Then, I will ask a few questions about what brings you in and what you are wanting to accomplish. If you choose to continue with case management services beyond our first meeting, we can keep meeting about your goals and needs, create steps to meet those goals, and then put your plan into action. This may look like many things, such as working together to build coping strategies, filling out an application for stable housing, connecting you with communities where you can build relationships, or role playing new situations, such as how to handle a job interview or how to have a challenging conversation with someone.

Whether you know what you need or are beginning a journey to discover what you want, you are not alone and we can figure it out together.
